Goal: Increase and improve the access and quality of care available to individuals at risk for diabetes and those with diabetes.
Objectives and Action Steps: 1) Promote adult diabetes guidelines 2) Develop health care provider guidelines for the screening, diagnosis, care and management of prediabetes 3) Identify and assist with the development of regulations or legislation to facilitate the provision of medications and education to individuals for the effective management of their diabetes 4) Work with employers to establish policies that provide employees with a work environment conducive to practicing the care required for diabetes 5) Improve access to health care services for children at risk for or with type 1 and type 2 diabetes by strengthening school-based health services 6) Develop guidelines for health care providers for the screening, diagnosis and management of type 2 diabetes in children and adolescents 7) Develop/identify uniform coding and reimbursement for the screening, treatment and management of people with risk factors for diabetes and prediabetes 8) Develop/identify a pool of medical interpreters with an understanding of diabetes issues who can be accessed by health care providers 9) Develop a model for a Massachusetts State Diabetes Collaborative, based on the Chronic Care Model, to increase access for people with diabetes to quality care and pilot the model in a minimum of five community health centers (CHC) not currently participating in the federal Health Disparities Collaborative
